ZechionMed (or “ZechionMed,” “Company,” “we,” “us,” or “our”) provides administrative support in medical billing, medical coding, revenue cycle management (RCM), claims management, denial management, payment posting and accounts receivable, credentialing support, and other healthcare administrative functions.
This Medical Billing & Coding Disclaimer provides information regarding the nature and the limitations of ZechionMed's Services. It's designed to set expectations for the healthcare providers, medical practices, healthcare organizations, and other Clients that use ZechionMed Services.
This Disclaimer should be read in conjunction with the applicable Terms & Conditions, Service Level Agreement (SLA), Master Services Agreement (MSA), Statement of Work (SOW), Business Associate Agreement (BAA), Privacy Policy, HIPAA Privacy & Information, Data Security & Privacy Policy and Refund & Cancellation Policy.
ZechionMed offers medical billing, coding, revenue cycle and healthcare administrative support services.
ZechionMed is not a medical service provider, nor a direct health care provider.
We do not take the place of the professional judgment or duties of:
Data made available by ZechionMed via its website, communication, reports, billing services, coding services or any other form of data is not medical advice.
ZechionMed does not:
Take over for a licensed health care provider.
Patients should seek advice from an appropriately qualified health care professional on medical questions or concerns.
Medical coding services are completed using information and documentation that is available to ZechionMed.
Coding may involve:
ZechionMed does not develop clinical documentation on behalf of a provider, and intentionally does not assign codes that are not supported by available clinical documentation.
The accuracy, completeness and authenticity of the underlying clinical documentation remains the responsibility of the healthcare provider.
Client is responsible for:
Filling out necessary information for coding and billing.
ZechionMed may highlight documentation issues or provide additional clarification wherever appropriate but the provider is still responsible for clinical documentation.
ZechionMed's coding activities are not a determination of:
Coding accurately documents services and conditions per coding requirements.
ZechionMed aims to offer accurate and compliant coding services, relying on the information made available to it.
ZechionMed, however, does NOT offer the following guarantees:
That they will receive a certain amount as a reimbursement.
Payer policies and interpretations may vary.
The determination of medical necessity is usually based on applicable law, the requirements of payers, clinical circumstances, medical documentation, and other applicable requirements.
ZechionMed does not set or ensure medical need for services provided by a health care provider.
The provider's responsibility to document services and provide medical justification for services continues.
ZechionMed reserves the right to prepare and file claims with the information supplied by the Client.
There are no guarantees for a claim submission:
Claims continue to be subject to payer regulations and adjudication.
Claim outcomes are determined independently by insurance companies, Medicare and Medicaid, government programs, clearinghouses and other payers.
Payers may:
ZechionMed has no control over the final determination by the payer.
ZechionMed has no guarantee of reimbursement of any claim.
You might have a claim that is denied or treated as low priority for the following reasons:
ZechionMed's RCM services aim to streamline healthcare revenue processes.
ZechiomMed does not guarantee, however, that:
There are many factors beyond ZechionMed's control which will affect actual financial performance.
ZechionMed can appeal and manage claim denials as per the scope of Services.
Denial management can involve:
There is no certainty of payment for an appeal or corrected claim.
If requested in the Client's contracted Services, ZechionMed can support for the client in handling payer appeals.
The success of an appeal is dependent on a number of factors, such as:
The level of appeal success cannot be guaranteed by ZechionMed.
If credentialing or enrollment support of providers is made available, ZechionMed may help with:
Applications for credentialing and enrollment are completed by the appropriate government agency, payer, or credentialing entity.
ZechionMed cannot make any guarantees:
If Eligibility Verification is part of the Services, information can be accessed from available Payer Systems or other authorized sources.
Eligibility will be subject to change.
Verification does not necessarily guarantee:
ZechionMed can assist with billing and account-management activities that include patient responsibility.
Patient balances may be adjusted, however, based on:
The Client shall be responsible for the development of suitable policies for patients' finances.
The Client must continue to give correct and complete information.
This can involve, when applicable:
ZechionMed may be unable to provide Services due to incomplete or inaccurate information.
The health care provider still has the responsibility of:
ZechionMed's aim is to provide the Services in compliance with applicable requirements relevant to the Services provided.

Zechiomed is not liable, however, for compliance of the Client's healthcare operation with any and all laws or regulations.
The Client shall be responsible for compliance with the organization's obligations.
If ZechionMed is a Business Associate under HIPAA, the execution of a Business Associate Agreement (BAA) and the application of the law governs use and handling of applicable PHI.
The BAA is not included in ZechionMed's public website policies.
The BAA sets forth certain duties with regard to:
ZechionMed may use patient information to provide services for Clients, as required to provide contracted Services.
In general, patients should ask questions about:

ZechionMed reserves the right to modify workflows as necessary and/or applicable, based on the information available.
But the interpretation of the payers can vary from coding guidance or industry standards.
Final claim adjudication could be impacted by applicable payer policies and/or contractual requirements.
ZechionMed may use third party systems such as:
ZechionMed is not responsible for the availability, functionality, processing times and/or decisions made by third party systems.
In some processes, ZechionMed may rely on technology, software, automation, analytics and AI-assisted tools.
If used, such technologies are designed to aid with approved business processes.
Technology is not a substitute for proper human review, professional judgment, Client responsibility or regulatory requirements.
Outputs may need review or validation to be used for claims, billing, coding or other workflows when AI assisted technology is used in an authorized workflow.
An output generated with the assistance of AI is not always guaranteed to be accurate.
ZechionMed reserves the right to take any necessary quality-control measures depending on the level of risk and service offered.
ZechionMed strives to deliver accurate and reliable Services.
But there are no billing, coding, technology or administration processes that can assure that there will be no mistakes.
If an error is detected, ZechionMed will research the error and make appropriate corrections as outlined in the agreement and workflow.
Audits or reviews may be conducted by the client:
When the contracted Services include billing or coding audit activities, ZechionMed may be able to help with applicable billing or coding audit activities.
The outcome of an audit depends on the entity that is reviewing the audit.
ZechionMed is allowed to keep records based on:
It is important to note that record retention does not mean that all historical records will be available indefinitely.
ZechionMed has security and privacy practices in place to safeguard information entrusted to it.

A system or communication method can never be considered 100% secure by any organization.
ZechionMed's Service Level Agreement sets operational targets for Services that are contracted.
The SLA targets do not guarantee:

Unless it is specifically stated that such examples, projections, estimates, case studies, performance indicators, or financial information have been incorporated by reference, they are examples of information provided by ZechionMed for informational purposes only.
ZechionMed does not guarantee a particular financial result.
A claim which has been prepared or reviewed by ZechionMed may still be:

Some ZechionMed Services need to have access to Client systems.
Factors that can impact the performance of service:
Where the issue is beyond ZechionMed's reasonable control, due to non-availability of Client systems, ZechionMed is not responsible for any delays caused by this.

A Client may have requirements which are different from ZechionMed's general processes.
Requirements for clients should be captured in the relevant:

ZechionMed is not an Emergency Medical Service.
If a medical emergency occurs, dial the proper emergency numbers or go immediately to the hospital.
Using the ZechionMed website or ZechionMed for Services, users acknowledge that:

The Clinical Documentation remains the responsibility of the Client; and
