Provider data is not filed once. Addresses change, providers move between locations, specialties are added, groups reorganise — and every one of those has to reach every payer, or the payer's record quietly stops describing the practice.

How Stale Data Shows Up
A directory patients cannot use
Wrong address or phone number in a payer directory means patients cannot reach the practice, and payers increasingly treat directory accuracy as a contractual obligation rather than a courtesy.
Denials for an unknown location
Claims from a location the payer has never been told about deny as out of network, from a practice that is contracted with that payer.
Payments to a closed account
EFT details that were not updated send remittance somewhere it cannot be received, and recovering it takes longer than updating it would have.
